Opportunity Description
As a DES Ongoing Support Consultant, your role will be to monitor and provide ongoing post placement support to participants in employment, identify barriers to ongoing employment and provide a solution focused approach to assist the participant in maintaining employment and achieving milestones.
This role is located in the Perth Central West Metro Region and report directly to the Team Leader.
What we need from you:
Supporting our job seekers to maintain their employment through regular phone and/or face to face contact
Submitting claim evidence and information that is timely and accurate
Identify alternative or additional training and/or support based on individual needs of our employed participants
Management of PPS caseload and adhering to all contract requirements
Think outside the box and resolve challenges for both parties as they arise
Help employers provide a suitable and positive work environment for our customers & guide on workplace modifications
Preparation for and involvement in Ongoing Support Assessments
Actively engage with customers to positively advocate the APM brand
Passing internal and external audits in line with Disability Employment Services contract

Eligible to work in Australia
Current driver’s licence
Access to a comprehensively insured vehicle for work purposes
